Please note this entire form will be submitted to NZ Police for completion of the police vet. The entire form will also be copied to the applicant's employing authority. This allows us to send the combined and integrated consent to disclosure with the application details.

Instructions for completion

• Part A must be completed by the non teacher applying for the police vet.
  
• Part B must be completed by the designated vet requestor of the institution that is requesting the vet.
